Measles makes a comeback: Canada loses ground, with the US on the brink.
In a concerning development, Canada has lost its hard-earned measles elimination status, a title it held for almost three decades. This news hits close to home for Americans, as the US is teetering on the edge of a similar fate. The Pan American Health Organization (PAHO) delivered the verdict to the Public Health Agency of Canada, stating that the country had failed to maintain the required standard for measles control.
A country's measles elimination status is at risk when it faces 12 consecutive months of measles transmission. Canada achieved this milestone in 1998, two years ahead of the US. PAHO Director Jarbas Barbosa acknowledged the setback but emphasized that it's not irreversible.
But here's where it gets controversial... The US is on the cusp of losing its own measles elimination status, having experienced some of the worst outbreaks in recent memory. Infectious disease specialist Isaac Bogoch from Toronto General Hospital pointed out that the reasons for Canada's loss are the same factors affecting the US.
The primary culprit? Declining vaccination rates. Health experts stress the importance of a 95% vaccination rate to keep measles at bay. However, the US saw a drop in kindergarten vaccination rates to 92.5% in the 2024-2025 school year, according to the CDC. In Canada, while overall immunization rates are high, there are regional disparities, with some communities falling behind.
And this is the part most people miss... Under-vaccinated communities, such as a Mennonite community in southwestern Ontario, are particularly vulnerable. When measles cases are imported into these areas, the virus can spread rapidly. This scenario played out in Texas this year, where a measles outbreak originated in a Mennonite community and led to over 1,700 confirmed cases nationwide.
While the Texas outbreak has been contained, other states like Arizona, South Carolina, and Utah are still battling active outbreaks. PAHO warns that if the US doesn't stop measles transmission by January 20, 2026, it will lose its elimination status, with Mexico potentially following suit. American infectious disease specialists believe it's inevitable.
Professor Andy Pavia from the University of Utah expressed his belief that the US will lose its elimination status due to the ongoing transmission. He also highlighted the broader implications, stating that it reflects a failure to contain outbreaks rather than directly affecting funding or travel restrictions. Pavia criticized the inadequate response to falling vaccination rates and the lack of effort to restore trust in immunizations.
PAHO acknowledged the loss of elimination status has no formal consequences but emphasized the potential for serious health and social impacts, including preventable deaths. Bogoch noted that while recent outbreaks in Canada were controlled, the technical definition of sustained transmission was met, damaging Canada's reputation as a high-income country with an advanced public health system.
In the US, losing the elimination status would have tangible effects. Pavia explained that sustained transmission and multiple outbreaks are the primary reasons for the potential loss. While some outbreaks have been contained, the challenge lies in preventing new outbreaks from emerging in different states.
The UK, another developed nation, lost and then regained its elimination status in 2019 and 2021, respectively. Canada is working towards the same goal. If the US loses its status, Pavia stressed the need to increase immunization rates, which would require better support from the Department of Health and Human Services. He also highlighted the importance of quickly containing outbreaks to prevent interstate spread, which would require additional funding for state and local health departments and CDC assistance.
